Indemnity Healthcare Insurance

Indemnity healthcare insurance is the first type of medical plan offered by insurance companies. They pay either a fixed amount for specific medical procedures (schedule of benefits) or a percentage of the healthcare provider's fee. The patient receives and pays for medical services, and the insurance company reimburses or indemnifies the insured based on the provisions of the insurance policy.

Major Medical Insurance

Major medical healthcare insurance is probably the most common type of health insurance plan. It provides benefits for a broad range of healthcare services based on usual and customary charges rather than on a schedule of benefits. Major medical insurance plans typically incorporate coinsurance, co-payments, coverage limits, deductibles and out-of-pocket maximums in the policy.

Health Maintenance Organization (HMO)

With the Health Maintenance Organization Act of 1973, Health Maintenance Organizations (HMOs) came into being. Benefits are provided through a network of providers. Unlike other forms of healthcare insurance, an HMO covers only care provided by member doctors, medical other professionals and facilities who have agreed to the terms and restrictions of the HMO.

Most HMOs require the insured to choose a primary care physician to limit access to medical services. Primary care physicians are usually general practitioners, internists, family doctors or pediatricians. Patients usually need a referral from the primary care physician in order to see a specialist or other doctor unless it's a medical emergency. It is the responsibility of the primary care physician to make sure the HMO guidelines are met prior to authorizing a referral.
